Summary:
The Nurse Case Manager is responsible for coordinating and providing comprehensive healthcare services to patients within a designated healthcare facility or organization. Responsibilities:

8. Facilitating patient transitions between healthcare settings (e.g., hospital to home, primary care to specialty care) to ensure continuity of care.
9. Identifying potential risks, complications, or barriers to care and implementing appropriate interventions.
10. Collaborating with insurance carriers, social workers, and other stakeholders to advocate for patients' healthcare needs and ensure optimal utilization of resources.

Qualifications:
1. Bachelor's degree in Nursing (BSN) from an accredited nursing program.
2. Current and unrestricted Registered Nurse (RN) license in [specify location].
